FFY 2013 Grant Awards

Congratulations on your FFY 2013 grant award!

To complete the award process, please find below a list of required documents categorized by funding source that must be completed, endorsed and returned to the Arizona Department of Homeland Security (AZDOHS) as explained in your award notification.

Please note that quarterly reports are due as indicated in your subgrantee agreement regardless of whether there is activity or not. Failure to submit quarterly reports may delay or halt processing of reimbursement requests.

State Homeland Security Grant Program (SHSGP) and Urban Area Security Initiative Grant Program (UASI)

  • Subgrantee Agreement (120.68 KB) (Complete and return two originals)
  • Environmental and Historic Preservation (EHP) Review
    Note: EHP Review must be completed prior to initiating any project that requires a review. The completion of the EHP Review is only required if you received a notification in your award information.  The notification, if you received it, delineated the level of review required for your project.  If you did not receive notification, you do not have to complete the Review.
