Filing a Discrimination Complaint

As a recipient of federal financial assistance, the Arizona Department of Homeland Security (AZDOHS) is required to comply with applicable provisions of laws and policies prohibiting discrimination. Discrimination based on race, color, religion, national origin (including language), age, sex, or disability is prohibited by law. If you believe you have been discriminated against with respect to applying for or participating in programs or activities from AZDOHS, you have 180 calendar days from the last act of discrimination to submit a complaint. Please complete the Discrimination Complaint Form below and submit it to AZDOHS via email, fax, or regular mail as described at the top of the Discrimination Complaint Form.

Reasonable Accommodations Request

Pursuant to Title VI of the Civil Rights Act of 1964, the Americans with Disabilities Act (ADA) and other nondiscrimination laws and authorities, the Arizona Department of Homeland Security (AZDOHS) does not discriminate on the basis of race, color, national origin, sex, age, or disability. Persons that require a reasonable accommodation based on language or disability should complete the Reasonable Accommodation Form below and submit it to AZDOHS via email, fax, or regular mail as described at the top of the Reasonable Accommodation Form. Requests should be made as early as possible to ensure the State has an opportunity to address the accommodation.

AZDOHS will generally, upon request, provide appropriate auxiliary aids and services to persons with disabilities, at no charge, to assist in effective communication and to participate equally in Department programs, services, and activities, including qualified sign language interpreters, documents in Braille, and other ways of making information and communications accessible to people who have speech, hearing, vision, learning, or other impairments.

AZDOHS will make all reasonable modifications to policies and programs, at no charge, to ensure that people with disabilities have an equal opportunity to participate in its programs, services, and activities. If you believe that you will not be able to understand or take part in a program or activity because of your disability, or need an auxiliary aid or service or a reasonable modification in policies or procedures, please let us know of your needs in advance if at all possible. To request an auxiliary aid or service, a modification of policy or procedure, to obtain this document in alternative format, or for further information about this policy, please email hs@azdohs.gov.

The ADA does not require AZDOHS to take any action that would fundamentally alter the nature of it programs, services or activities, or impose an undue financial or administration burden on the Department.
